Immunization Information
2018-2019 Immunization requirements for students KG-5th grade:
DTaP- Four (4) or more doses- including one since age 4.
Polio- Three (3) doses including one since age 4.
MMR- Two (2) doses after 1st birthday.
Hepatits B- Three (3) doses.
Varicella (Chickenpox)- Two (2) doses received on or after 1st birthday. Note from parent or doctor verification of illness is required in lieu of vaccine.
Hepatitis A- Two (2) doses after 1st birthday or serologic confirmation of immunity.
Immunization exemptions must be presented to the nurse before a student can start school. Only original forms are accepted, no copies. Exemptions are valid for 2 years.
