Medication Pick Up for End of School Year

All medication must be picked up from the nurse's office before the last day of school. Medication will not be stored in the clinic over the summer. All medication left will be disposed of the last day of school.

 

If you prefer to have your child bring the medication home, please download this form, fill it out, and return it to the school nurse before the last day of school.
