Welcome to the San Jacinto Elementary Clinic Information page. Below you will see the minimum state vaccine requirements and to the left you will find links to answers of the most frequently asked questions. MINIMUM STATE VACCINE REQUIREMENTS FOR TEXAS CHILDREN ENTERING KINDERGARTEN:
1. Polio (IPV/OPV) - 3 doses including one received on or after
the 4th birthday
2. DTAP/DTP - 4 doses including one on or after the fourth birthday
3. Hepatitis B - 3 doses
4. Varicella - Two doses received on or after 1st birthday
or written affidavit that the student has had the chickenpox
disease
5. MMR - 2 doses
6. Hepatitis A - 2 doses
