Polio: Three doses, including one received on or after the fourth birthday. DPT: Three doses, including one received on or after the fourth birthday, and one dose every 10 years. MMR: Must have 2 doses, both must be on or after the first birthday, and must be at least 30 days apart. Hepatitis B: Must have 3 doses. Varicella: If given at age 1 through 12, one dose. If given over age of 13, 2 doses required, with 4-6 weeks between doses. A parent or doctor note verifying illness may be used in lieu of vaccine.

Medication
at School No medication shall be administered unless the medication appears in the original container and is properly labeled. No foreign medications will be accepted. All medications must be FDA approved. No anabolic steriods, dietary supplements, or herbal substances will be administered except if required by the IEP or Section 504 plan. We encourage parents to schedule the administration of student medicine in such a manner that medication brought to school will be kept to a minimum. |
