Medication at School
If your child is to take medication during school hours, the medication must be brought to the clinic as soon as the student arrives on campus. No medication shall be administered unless the nurse receives a written request from the parent, legal guardian, or other person having legal control of the student. The request must state the following: student's name, name of the medication, date, time, and the dosage.
No medication shall be administered unless the medication appears in theoriginal container and is properly labeled. No foreign medications will be accepted. All medications must be FDA approved. |
Immunization Requirements:
Polio: Three doses, including one received on or after the fourth birthday.
DPT: Three doses, including one received on or after the fourth birthday, and one dose every 10 years.
MMR: Must have 2 doses. One after the first birthday - the second dose at least 30 days later.
Hepatitis B: Children born after 09/02/1988 must have 3 doses.
Varicella (chicken pox): Children born after 09/02/1988 must have vaccine. A note from doctor verifying illness is required in lieu of vaccine. |
