Enrollment Procedures:

1. Parent/Legal Guardian must be present
2. Proof of Address - Deed or payment book, Lease Agreement, No Bills.
3. Last Report Card
4. Social Security Card
5. Birth Certificate
6. Shot Records
7. Withdraw Paper (if during school year)

Withdrawal Procedures:

1. Parent/Legal Guardian must be present to sign all paperwork.
2. All textbooks must be returned

Requesting Assignments During Absences:

1. Absent 3 or more days.
2. Parent may phone counselor's office to request the student's homework assignment.
