MINIMUM STATE VACCINE REQUIREMENTS FOR TEXAS  CHILDREN ENTERING  KINDERGARTEN:
 1. Three doses polio vaccine, including one received on or after the 4th birthday
 2. Four doses DTP/DTaP vaccine, including one received on or after the 4th birthday
 3. Three doses hepatitis B vaccine
 4. One dose varicella vaccine received on or after 1st birthday or written affidavit that  the student has had the chickenpox disease

MEDICATION AT SCHOOL:
No medication shall be administered to a student unless the medication appears in the original container and is properly labeled. NO FOREIGN MEDICATIONS WILL BE  ACCEPTED. All  medications must be FDA approved.

No employee shall administer any medication to a student unless the district has  received a written request from the parent, legal guardian, or other person having legal control of the student to administer the medication. The request must state the following: student's name,  name of medication, date, time, and dosage.

 GUIDELINES FOR COMMUNICABLE DISEASES:
Children with an infectious or contagious disease should stay home until the danger of transmitting the disease has passed.

FEVER: (100.4 DEGREES or greater): student must be free of fever for 24 hours before returning to school.

STREP SORE THROAT: re-admit after 24 hours from date antibiotic treatment started.

HEAD LICE: re-admit when student is free of lice and nits.

VOMITING or DIARRHEA: your child should stay at home if they have these symptoms during  the night or in the morning.

PINK EYE (CONJUNCTIVITIS): re-admit with doctor's written statement.
