Minimum State Vaccine Requirements
DTP – Four (4) or more doses - including one since age 4 AND
Grade 7 – A booster dose of a tetanus/diphtheria/pertussis-containing vaccine if 5 years have passed since the last dose of tetanus-containing vaccine. Td vaccine is acceptable if Tdap is medically contraindicated.
Grades 8 – 12 – A booster dose of a tetanus/diphtheria/pertussis-containing vaccine if 10 years have passed since the last dose of tetanus-containing vaccine. Td vaccine is acceptable if Tdap is medically contraindicated.
Grades 8 – 12 – A booster dose of a tetanus/diphtheria/pertussis-containing vaccine if 10 years have passed since the last dose of tetanus-containing vaccine. Td vaccine is acceptable if Tdap is medically contraindicated.
POLIO – If under age 18, three (3) doses including one since age 4
MMR – Two (2) doses after 1st birthday
HIB (Haemophilus Influenza Type B) – Three (3) doses for all children age 18 months through 4 years
HEPATITIS B – Three (3) doses
VARICELLA (Chickenpox) – Two (2) doses received on or after their 1st birthday.
(Note from parent or doctor verification of illness is required in lieu of vaccine.)
HEPATITIS A – Two (2) doses after the 1st birthday (required for all students entering PPCD, Pre-K - 8th grade in 2017-2018)
PCV7 OR PNEUMONIA CONJUGATE OR PREVNAR – Two (2) doses after 1st birthday

MENINGOCOCCAL -
One (1) dose of quadrivalent meningococcal conjugatevaccine is required on or after the 11th birthday for studentsentering 7th - 12th grade in 2017-2018 Note: If a studentreceived the vaccine at 10 years of age, this will satisfy the requirement.

The LaPorte Fire Station #2, located at 10428 Spencer Hwy in LaPorte, gives immunizations free of charge on the second Saturday of every month, from 9:00am to noon. A parent must be present for the child to receive any immunizations, and you must bring a copy of your child’s immunization record with you. Their phone number is 281-471-9244.
