Deer Park ISD Athletic Department exclusively uses Rank One Pro as our comprehensive athletic management system. Rank One Pro offers new features to allow parents and athletes to access their account, schedules, and more from their phone, tablet, or PC. Parents are now required to sign in so the advanced capabilities can function seamlessly.

The UIL has updated the Pre-Participation Medical History and Physical Examination for 2026-2027. You MUST use the new "2026" form, as older versions may be denied when uploaded.

UPLOADING ATHLETIC PHYSICAL IN RANK ONE

You may upload their child's physical by submitting either a PDF file or an image taken on your phone, tablet, or PC. If uploading an image, ensure the form fills most of the screen and is clear, flat, in focus, and unobstructed. If you upload multiple pages, upload each page as a separate image. After uploading the first image, click the "Select Image" box again to add the next image. Once submitted, the DPISD athletic trainers will review the form and either approve or deny it. If the form is denied you will receive an email notification that it was denied.
Before clicking SUBMIT, make sure the file has fully uploaded. The file should appear in the system and be marked in GREEN. If you submit before the upload is complete, the athletic trainers will not be able to view the document, and the system will send you a generic email stating that the upload was denied.
When completing the online forms, you MUST SELECT A SPORT. This ensures you receive the correct forms required for participation. The system only displays forms for the sport or activity your child plans to try out for or participate in. Forms also differ between junior high and high school students, so please be sure the correct sport and level are selected.
If you receive a denial email and are unsure why, please email the athletic trainers at [email protected] with your child's name and student ID number. We will respond as soon as possible to assist you. Most denials occur when the file was submitted before it finished uploading.
If you are unable to upload the physical exam, you can email the form to us at [email protected] or return the originals to the athletic trainers at the South Campus. For your convenience, a 24-hour drop box is located outside the South Campus Athletics doors #9.

PRE-PARTICIPATION PHYSICAL EXAM FOR 2026-2027

ANNUAL PHYSICAL REQUIRED - All students participating in athletics, cheer, dance, or high school marching band must complete a NEW physical examination EACH YEAR before tryouts, practice, or competition.
ATHLETIC PERIOD REQUIREMENT - All required athletic paperwork must be submitted BEFORE participating in an athletic period or any tryouts, even if tryouts for that sport occur later in the school year.

RANK ONE ATHLETIC FORMS ARE SPORT & ACTIVITY SPECIFIC

FORMS ARE SPORT & ACTIVITY SPECIFIC: When completing forms in Rank One, you will only see the forms required for the sport or activity your student plans to tryout for or participate in. Forms differ between junior high and high school sports and activities.
ONLINE FORMS: All required forms are completed and submitted online, with the exception of the Pre-Participation Physical Exam.
PHYSICAL FORM: The physical form must be printed and taken to the physician of your choice. Many physicians prefer to review the medical history form during the exam, so it will print with the physical form. You may return the printed medical history form; however, you MUST complete the online medical history form to be eligible.
CURRENT UIL FORM REQUIRED: Physicals must completed on the current 2026 UIL Pre-Participation Physical Exam form. Physicals submitted on older UIL forms or forms from other organizations will not be accepted. Download the current Pre-Participation Physical Exam form HERE or access it under the "Downloadable and Printable Forms" tab in Rank One.
DATE REQUIRED: All forms for the 2026-2027 school year must be dated April 1, 2026 or later.

COMPLETE ONLINE FORMS AND SUBMIT PHYSICAL

Complete all required online forms through the Rank One Parent Portal and upload your child's physical directly into Rank One using any of the following methods:

DOWNLOAD THE RANK ONE PUBLIC APP

Download the Rank One Public app by going to the Apple App Store or Google Play Store on your phone or tablet. Create or log in to your account to track whether your child has completed all required forms for eligibility in UIL sports & activities. You can also sign up for schedule updates and other notifications through the R1 Public app.

DIRECTIONS FOR USING THE RANK ONE PARENT PORTAL

ONLINE FORMS:
[MUST BE COMPLETED THROUGH THE RANK ONE PARENT PORTAL]

LOG IN OR CREATE AN ACCOUNT: Login, create and account, or continue as a guest.
SELECT SPORTS & ACTIVITIES: Choose the sports or activities that your child plans to participate in this year. If a sport is not listed, it has already been selected by the school.
START FORMS: Click "START FORMS", then select each form and complete the requested information. If you are unsure of an answer, write "Not Applicable (N/A)" or "Unknown".
SIGN FORMS ELECTRONICALLY: Click inside the signature box and hold down your mouse to create an electronic signature. To start over, click on the refresh icon next to the signature box.
PRINT FORMS (OPTIONAL): After completing a form, you may print a copy for your records. Do not submit any printed forms except the physical. You can always login later and view or print forms.
CONFIRMATION: Once the DPISD athletic trainers have reviewed your submitted documents, a confirmation email will be sent if you provided an email address at the end of each form.
