UPLOADING ATHLETIC PHYSICAL IN RANK ONE
 
Parents can upload an image or PDF of their child's athletic physical directly to Rank One for  the athletic trainers to review.
 
Before clicking SUBMIT, please make sure the file has fully uploaded.  The file should appear in the system and be green.  If you submit before the upload finishes, the athletic trainers will not be able to see the document, and the system will send you a generic email saying the upload was denied.
 
When completing the online forms, you MUST SELECT A SPORT first.  The ensures you receive the correct forms required for participation.  They system only shows forms for the sport or activity your child plans to try out for or participate in.  Forms also differ between junior high school and high school students, so please make sure the correct sport and level are selected.
 
If you receive a denial email and are unsure why, please email the athletic trainers at [email protected] with your child's name and student ID number.  We will respond as soon as possible to help.  Most denials happen when the file was submitted before it finished uploading.

ATHLETIC PAPERWORK UPDATE - Updated for 2026-2027

PRE-PARTICIPATION PHYSICAL EXAM FOR 2026-2027
  • ANNUAL PHYSICAL REQUIRED - All students participating in athletics, cheer, dance, or high school marching band must have a NEW physical examination EACH YEAR prior to tryouts, practice, or competition.

  • ATHLETIC PERIOD REQUIREMENT - All required athletic paperwork must be submitted BEFORE participating in an athletic period or any tryouts, even if tryouts for that sport occur later in the school year.
 
RANK ONE ATHLETIC FORMS ARE SPORT / ACTIVITY SPECIFIC
  • FORMS ARE SPORT/ ACTIVITY SPECIFIC:  When completing forms in Rank One, you will only see the forms required for the sport or activity your student plans to tryout for or participate in.  Forms differ between junior high and high school sports and activities.
  • ONLINE FORMS:  All required forms are completed and submitted online, except for the Pre-Participation Physical Exam.
  • PHYSICAL FORM:  The physical form must be printed and taken to the physician of your choice.
  • CURRENT UIL FORM REQUIRED:  Physicals must be on the current 2026 UIL Pre-Participation Physical Exam form.  Physicals completed on older UIL forms or other organization forms cannot be accepted.  Download the current version of the Pre-Participation Physical Examination HERE or from "Downloadable and Printable Forms" in Rank One.
  • DATE REQUIRED:  All forms for the 2026-2027 school year must be dated April 1, 2026 or later.

A FASTER, MORE SECURE WAY TO SEND THE PHYSICAL
  • Parents can upload their child's Pre-Participation Physical Examination directly to Rank One.  You may upload photo taken by your phone or tablet or a scanned PDF file.  Photos must be clear, in focus, full resolution, taken on a flat surface, and free of background clutter so all information can be easily read.  Once submitted, the DPISD athletic trainers will review the form and either approve or deny it.  If the form is denied you will receive an email notification that it was denied.

  • Complete the required online forms through the Rank One Parent Portal and be able to upload your child's physical directly into Rank One using any of the following methods:

    3. Scan the QR code below to take you to the Rank One Sport Parent Portal and follow the directions found there.

DOWNLOAD THE RANK ONE PUBLIC APP
  • Download the Rank One Public app by going to the Apple Play or Google Play Store on your phone or tablet.  Create / login to your account.  You can keep up with if your child has completed all the required forms in order for them to be eligible for sports / activities.  You can also sign up for notifications in your child's schedule and other notifications through the R1 Public app.

DIRECTIONS FOR USING THE RANK ONE PARENT PORTAL 
 
ONLINE FORMS:
[MUST BE DONE THROUGH THE RANK ONE WEBSITE PARENT PORTAL]
  1. After reviewing these directions, proceed to online forms.
  2. Login and CREATE AN ACCOUNT or continue as a guest
  3. To ensure you receive the correct forms, select the sports/activities that you expect your child to participate in this year.  If a sport is not listed, it has already been selected for your child by the school.  
  4. Click "Start Forms".
  5. Click on the form name and fill out the information requested.  If you are unsure about the answer to a question, please answer not applicable or unknown.
  6. To sign the document, click inside the signature box and hold your mouse down.  This will allow you to create an "Electronic Signature."  If you make a mistake and need to start over, click on the refresh icon next to the signature box.
  7. Once you have completed the form, you will have the opportunity to print the document.
  8. You will receive a confirmation email once the documents have been reviewed by the DPISD athletic trainers.

PRE-PARTICIPATION PHYSICAL EXAMINATION FORMS:
[MUST BE DONE THROUGH THE RANK ONE WEBSITE PARENT PORTAL]
  • ALL athletes, cheerleaders, drill team members, and high school marching band members must obtain a physical annually.
  • The physical form must be printed from the download and print tab and taken to the physician of you choice for completion.
  • Many physicians prefer to review the medical history form when they administer a physical exam and for this reason, the medical history form will print with the physical.  You may return the printed medical history form but you MUST complete the online medical history form to be eligible.
  • Physicals must be completed on the current UIL physical form.  Physicals completed on outdated UIL forms or other organization's forms cannot be accepted.
  • Once completed, it is recommended that you upload the form using the "Physical Upload Form" tab on the forms page.  Uploading the physical is the quickest and most secure way of getting the physical to us.
  • To upload the physical, follow these steps:
    1. Select "Physical Upload Form".
    2. Select how you would like to upload your document.  You can either upload a PDF file or an image file taken on you phone or tablet.  If uploading an image, make sure the form takes up most of the screen and the image is clear, flat, in focus, and unobstructed.  If uploading more than one page, each page should be a separate image.  after uploading the first image, click the "Select Image" box again to upload the second image.
    3. Complete the name and signature section.
    4. After submitting, the athletic trainers will review and either approve or deny the physical.  If the physical is denied, you will receive an email notification
  • If you are unable to upload the physical exam, you can email the form to us at [email protected] or return the originals to your child's coach or the athletic trainers at the South Campus.  For your convenience, there is a drop box located outside the South Campus athletic office doors which is accessible 24 hours per day.
